Juvenile Diabetes Research Foundation Request for Volunteer Performers

Dear Performer,
On August 2, 2003, we are hosting "Jumpers for Jesse", a fundraiser for the
Juvenile Diabetes Research Foundation. The money we raise will go directly
towards finding a cure for diabetes. Diabetes affects over 16 million
Americans, including our 4 year old nephew, Jesse Rimland. We are holding
this event in his honor.
Attendees will enjoy an afternoon of activity, socialization and fun for
invited residents of Warren and the surrounding area. Money will be raised
through a raffle of donated goods and services and through sponsors of a
basketball shootout.
We are currently soliciting donations from area businesses, for prizes,
entertainment and refreshments. We would appreciate your support and
welcome your donation to this project. Specifically, we are requesting a
donation of entertainment from you for our event.
The donation qualifies as a tax deductible charitable contribution.
This letter and our letter of thank you indicating your exact donation are
sufficient documentation of your donation for tax purposes. Please complete
the attached donation certificate and return it to us by July 1, 2003.
